ACO Network Development

  • Executed a 21 day “sprint” network development project for an ACO Reach client to round out their filing for the August 1, 2023 deadline. Our team secured over 15 contracts with primary care providers that led to the ACO adding an additional ~5,000 attributed lives to their plan. This client is a next-generation Primary Care Provider growing rapidly in towns and cities around the country. They are currently available to Medicare patients exclusively, and operate fully at risk for all of the patients’ outcomes and spend.

National ISNP Plan – Gap Fill Network Build

  • Currently working on a national gap fill network build with the nation’s largest ISNP Health Plan. 
  • The current focus is on seven (7) counties in the targeted service area to cure over 300 gaps.
  • Network development inclusive of hospitals, individual and large professional organizations, behavioral health facilities and ancillary service providers.
