This policy describes how Blakemore Consulting, LLC handles information submitted through blakemore-consulting.com.
Scope. This policy covers this website only. Information handled during a client engagement is governed by the executed services agreement and any applicable business associate agreement, not by this policy.
The contact form on this site collects the name, organization, work email address, and message you provide. We do not ask for and do not want protected health information, member data, or provider data through this form.
Our host records standard server information such as IP address, browser type, and pages requested. This site loads typefaces from Google Fonts, which means your browser makes a request to Google when the page loads. We do not use advertising cookies, tracking pixels, or third-party analytics scripts.
To respond to your inquiry and to conduct the resulting business correspondence. We do not use this information for anything else.
This site is hosted by Netlify, which also receives and stores contact form submissions on our behalf. Form notifications are delivered to a Google Workspace mailbox. Both act as service providers to us and process the information in order to deliver the service.
Form submissions are retained in our host's dashboard and in our email for as long as needed for the correspondence and for our business records. You may ask us to delete your information at any time.
Write to brooks@blakemore-consulting.com to request a copy of what we hold about you, to correct it, or to have it deleted. We will respond within a reasonable period.
Depending on where you live, you may have additional rights under state or other privacy laws. We honor those requests regardless of whether a particular law applies to us.
This site is served over HTTPS. We take reasonable measures to protect information submitted through it, though no method of transmission over the internet is completely secure.
If this policy changes, the updated version will be posted here with a revised date.
